Membership Change Form

Do you need to make changes to your membership? With the form below you can Freeze, Update Address and contact information, or Cancel your membership. Also there is a comment field and we appreciate all the feedback we can get.

Need to update your payment information? This form is not encrypted so please do not include any credit card or checking account information. Please call the gym or stop by to give us your new payment information.

Policies

  • EFT transactions occur on the 20th of each month or the first business day thereafter.
  • Any changes to EFT memberships must be submitted a minimum of three business days prior to the 20th.
  • EFT memberships are open ended and will continue until cancelled.
  • All freeze and cancellation requests will be confirmed by email.

Freezes

  • Freeze requests must be submitted three business days prior to the billing on the 20th of the month. We will make every effort to freeze your membership as soon as possible but we cannot guarantee that we can accommodate request made after this date.
  • All freezes start on the 20th of the month.
  • We bill $5 per month per member to freeze. Please list all family members who wish to freeze in the comments field.
  • You can freeze for as long as you want and unfreeze at any time.
  • Unfreezing on a date other than the 20th may result in a pro-rated membership charges for EFT members.

Cancellations

  • Cancellation requests must be submitted three business days prior to the billing on the 20th of the month. We will make every effort to cancel your membership immediately but we cannot guarantee that we can accommodate request made after this date.
  • Please list all family members who are cancelling in the comments field.
